Insurance Information

The practice is not able to accept clients with Medicare/Medicaid insurance coverage, workers' compensation or those clients filing for disability.

Please check with your insurance plan for out-of-network provider reimbursement if insurance coverage differs from listed plans.

Cost of services are $350 for initial evaluation and $200 for follow up appointments.
